Animal intake

This form is intended for organizations that would like to entrust us with a dog or cat in need of medical care in order for us to offer it a chance. Our team of volunteers will get back to you as quickly as possible. Your request will be evaluated in a timely manner, and we will contact you to let you know if we can take the animal in. If we cannot take the animal in, we will make it our mission to refer you to one or more organizations that could help you.
